If you are a new patient, please print and fill out the forms in the first column. If you are being seen for dizziness or pelvic floor issues, please fill out the additional form in the second column. Bring these forms with you for your first appointment. This allows us to spend more time evaluating and treating you.
Insurance and Cash Rates
We accept most insurance plans EXCEPT Medicare (Red, White and Blue card), DSHS, and United Healthcare.
We DO accept most Insurance including MedAdvantage plans.
If you would rather pay without going through insurance, we offer discounted cash rates for 30, 45 or 60 minute appointments.

If you are using insurance, it is your responsibility to know what your copay is and the insurance company's policy regarding Physical Therapy. Please speak with them prior to starting PT to know your financial responsibilities.
What To Expect On Your First Visit

At the first visit you'll need to bring in your insurance card, photo ID, and your copay (if this is part of your insurance plan). Along with this information you'll also need to bring in the completed new patient evaluation form (found on our website) and any new or ongoing prescriptions from your doctor.

Your therapist will complete an evaluation of your injury or condition. They will then determine a treatment plan which begins at the first visit. Please arrive 10 minutes before your first appointment to complete your new registration. If you have any time constraits, please let your therapist know at the beginning of your session.

We suggest you wear a loose t-shirt or tank top, shorts or yoga pants made of a soft, lightweight material that is easily stretched (if you can’t do a butterfly stretch in them they are too tight). Wear or bring the shoes you exercise in or the pair that is causing you discomfort.
